incoherent speech. This may be speech in which ideas shift from one subject to another seemingly unrelated subject, sometimes described as loosening of associations. Other types of disorganized speech include responding to questions in an irrelevant way, reaching illogical conclusions, and making up words. See neologism; paralogia.

a drawn or mechanically generated outline, often a graph, representing the symptoms and characteristics of a disorder.
