person-centered team

a group of people who meet periodically to develop plans for supports and services to enhance the lifestyle and self-determination of an individual with a disability (e.g., an intellectual or developmental disability). The team uses methods based on principles of person-centered planning. Team participants are invited by the person with the disability or his or her advocate, rather than by a service organization or agency, and they need not be trained professionals in human services.
