human error

an error in the operation of a human–machine system resulting from human action or inaction (see error of commission; error of omission), as opposed to mechanical failures or faults in product or system design. Although human error can never be entirely eradicated, good design will minimize both its occurrence and its consequences (see exclusion design; fail-safe).
