assertive community treatment
a service delivery system for individuals with severe mental illness that includes a range of treatment, rehabilitation, and support services provided by a mobile multidisciplinary team comprised of, for example, a psychologist, a psychiatrist, a social worker, a case manager, and a nurse. The team establishes a close, consistent relationship with each individual and delivers services and supports to him or her at home and work.
