symptom checklist

any self-report inventory used to assess the severity of symptoms of a given disorder, as in the Beck Depression Inventory, or across a range of disorders, as in the Symptom Checklist-90–Revised. Symptom checklists are often used in both clinical and research settings to measure treatment progress and outcomes.
